Our story

Care shaped by the people most often overlooked

Our founding story is centered on a commitment to making compassionate, accessible, and evidence-based mental health care available to individuals and families across Maryland.

Chance For Life was founded by Oluwakemi Fagbuyi, a board-certified Psychiatric Mental Health Nurse Practitioner with extensive experience caring for children, adolescents, adults, and older adults — including those who are medically complex. That experience — across some of the most vulnerable and often overlooked populations — shaped a simple belief: everyone deserves a chance to live a life of purpose, peace, and hope, regardless of the complexity of their circumstances.

Oluwakemi Fagbuyi, MSN, PMHNP-BC

Founder, Chance For Life Inc.

Oluwakemi Fagbuyi is a board-certified Psychiatric Mental Health Nurse Practitioner dedicated to compassionate, evidence-based mental health care for children, adolescents, adults, and older adults. With years of nursing and leadership experience caring for medically complex individuals, she understands the connection between physical health, emotional well-being, and quality of life.

As founder, she is committed to creating a safe, supportive, and judgment-free environment where every individual feels heard, valued, and empowered — providing comprehensive psychiatric evaluations, medication management, and personalized treatment plans through telehealth and home visits.

Her mission is to help individuals find hope, healing, and a renewed chance for life through care that promotes resilience and lasting wellness.
